2016-17 USD 393 ACTIVITY PASS FEES

FOR ALL JUNIOR HIGH AND HIGH SCHOOL HOME EVENTS

 

All-Season Family Pass – $135

All-Season Single Pass – $85

                   Solomon USD 393 Walk-In Enrollment

Dates / Times: Monday, August 1 (Noon – 7pm), Tuesday, August 2 (8am – 4pm)

Location: Solomon High School Commons (Lower west entrance of the building)

For: All families with students attending USD 393 during the 16-17 school year (PreK-12th Grade)

Access to Paperwork: To access the enrollment paperwork, click on the Parents tab at the top of the website, then click on 2016-17 Enrollment Paperwork.  Forms can be completed in advance and brought to enrollment or filled out at enrollment.

Special Note: Returning students do not need to complete the student registration form.  We will have a PowerSchool print-out from last school year.  Any changes can be made on that form.

_____                 USD 393 Random Drug Testing Policy (only for students grades 6-12, read the policy and initial, 6th – 12th grade students must have a parent signature to participate in extra-curricular activities)

_____                 Google Apps for Education Permission Form (read policy and initial, this allows students to have school issued email accounts through Google)

_____                 Bus Transportation Information (only for students outside city limits who need bussed)

_____                 Application Packet for Free and Reduced Lunches (Families who qualify based on income)

_____                 Physical Examination Form (6th-12th students participating in athletics, exam must be performed and signed by a doctor)

_____                 KSHSAA Concussion and Head Injury Release Form (6th-12th students participating in athletics, read and turn in form)

_____                 Request for Medication to Be Administered (only for students taking any prescribed or over the counter medication at school)

_____                 Custody Law Notification (only for families experiencing custody disputes that the district needs to be made aware of)

_____                 Home Language Survey (only for students / families where English is not the primary language spoken at home)

_____                 Out of District Application (only for students who live outside the USD 393 district boundaries, see map on following page)

With the input of more than 2,000 Kansans, the Kansas State Board of Education has set a new Vision for Kansas education where “Kansas leads the world in the success of each student.” 

 

Kansans are demanding higher standards in academic skills, as well as employability and citizenship skills, and the need to move away from a “one-size-fits-all” system that relies exclusively on state assessments from No Child Left Behind. This new vision for education calls for a more student-focused system that provides support and resources for individual success and will require everyone to work together to make it a reality.
